Confidential Emergency Information Form

If you are a new business owner or have recently moved your business to a new location within the City of Roseburg, we would like to encourage you to complete or update a Confidential Emergency Information (CEI) form.  The CEI is a form which, once completed by local business owners, provides essential information to the Fire Department, Police Department and Douglas County Emergency Dispatch.  The information you provide on the form includes emergency contact names and telephone numbers, which Police and Fire personnel can use to contact a responsible person in the event of an emergency at your business after normal business hours.  Your completion of this form assists emergency responders in safeguarding your business and property.

You can obtain the CEI form by stopping by any Roseburg Fire station or the Community Development Department in Roseburg City Hall.  Another option is to download the form and fill it out, Confidential Emergency Information Form.  You can then either drop the completed form off at the Roseburg Fire Department at your convenience, or fax it to us at (541) 492-7046.  If you have any questions, please feel free to contact Deputy Fire Marshal Monte Bryan at (541) 492-6770.
